Employee - Lifestyle Companions Role Description

Summary of Responsibilities:
  • To be familiar and comply with Mix It Up - Live life your way’s policies and procedures.

  • To identify participants needs or changes and report them back to the Mix It Up- Live life your way Lifestyle Consultant.

  • To report and document all hazards and incidents within the required time frames.

  • To follow participant’s individualised support and activity/event plan.

  • To communicate effectively and relate to people with a disability, their families and/or advocates and work colleagues.

  • To support participants to achieve their desired goals and potential in relation to their emotional and social wellbeing.

  • To maintain and adhere to Mix It Up - live life your way’s Code of Practice and Code of Conduct.

  • To facilitate collaborative and professional working relationships within and external to the business.

  • To identify training needs and undertake training programmes.

  • To adhere to all relevant legislation including the Disability Act and perform responsibly under the Work, Health and Safety Act.

  • Follow and provide feedback regarding Mix It Up - Live life your ways policies and procedures thereby contributing to a safe and accountable work environment.

  • To manage information effectively by reporting and recording by utilising the documentation processes and procedures.

  • Ability to work with and accept cultural, racial and social diversity.

Working Knowledge of:
  • Participants Rights and Responsibilities.

  • Disability Service Standards.

  • Duty of Care Principles.

  • Privacy and Confidentiality Principles.

  • National Disability Insurance Scheme.

  • Possess sound knowledge of and consult with others in regard to Duty of Care when providing support to participants.

  • To provide one to one social support to Mix It Up - Live life your way participants.

  • Follow the Participant’s Individualised Support and Activity/Event Plan and contribute to reviewing the plan on an ongoing basis.

  • Report and document any participant changes and needs.

  • Empower participants by ensuring participant engagement in personal decision making.

  • Communicate and relate effectively to participant’s, families, advocates, and colleagues.

  • Maintain accurate financial records for yourself and the participant (when assistance is required).

  • To provide participant transport via personal vehicle, public transport or access cabs.

  • Identify training needs

  • Contribute to the review of Mix It Up - Live life your way’s policies and procedures.

  • Other duties as directed.

Reporting working relationship:
  • Lifestyle Companions report to the Lifestyle Consultant (Mix It Up- Live life your way Business Owner)

  • This position is subject to a 6-month probationary period. Starting from the date of employment.

  • This position is paid as per the Social and Community Services Award [AN150140] (SA)

  • This is position is paid at a level 2 classification. The pay point is determined by your qualifications, experience, and responsibilities.


Essential Requirements:
  • Flexible working hours – work over 7 days, after hours and weekends.

  • Willing to work in a variety of community settings.

  • Provision of satisfactory DCSI Aged services employment screening, Disability services employment screening, Child- Related services employment screening and vulnerable persons screening prior signing contract.

  • ‘C’ Class Driver’s License.

  • Full Comprehensive Vehicle Insurance.

  • Certificate III in Aged Care, Disability Services or Mental Health or higher – essential.

  • Senior First Aid Certificate - Current.

  • Good interpersonal skills in order to interact positively with participants and their support network, team members, other professionals and members of the public.

  • Effective written and verbal communication.

  • Identify and solve problems of limited complexity.

  • Work as a team member and abide by the Code of Practice.

  • Perform duties within established practices and guidelines.

  • To manage time effectively.

  • Work, Health and Safety requirements.

  • Previous experience in the Community Services Field.

  • Experience working as part of a team.

Work Health and Safety:
  • Observe all Work, Health Safety and Welfare policies and procedures, follow directions and report all WHS matters.

  • Conform to Mix It Up - Live life your way’s Code of Practice, Privacy and Confidentiality and all other policies and procedures.

  • Adhere to legislative requirements including but not limited to the WHS Act 2012, the Disability Discrimination Act and the Equal Opportunity Act.
