During your association with Mix It Up – Live life your way, personal information will be used to inform decisions about the services you receive to promote your health, wellbeing and lifestyle.

Workers/Sub-contractors.

On initial application, a form is to be completed by prospective workers/sub-contractors authorizing the collection of necessary personal information.

​

Notwithstanding that workers records are exempt from the Privacy Act, employee information held by Mix It Up – Live life your way will be maintained confidentially and securely, only used for appropriate purposes and will not be disclosed to external parties.

Mix It Up - Live life your way Pty Ltd. complies with the applicable privacy legislation and has systems for the collection, use and disclosure, quality, security, accuracy and correction of personal information relating to you as our participant. The privacy and dignity of participants will always be maintained. All participants will be asked to sign the Privacy Agreement to gain consent regarding the collection, use and disclosure of your information, to comply with the Privacy Act 1988.

This information is in our Privacy and Confidentiality Policy and is detailed within a section in the Participant Service Agreement.

Requests for access to the personal information we hold should be made in writing to the Director.
