The Mix It Up - Live life your way Policy on Client Incidents is:

• Mix It Up – Live life your way will support the development of safer environments for participants to live and participate in the community, focussing on risk management to avoid adverse outcomes to services, client injury prevention, minimisation of incident reoccurrence and monitoring of client outcomes.

 

• All participant incidents will be reported, documented and recorded.

 

• Mix It Up – Live life your way will comply with all requirements to report incidents to appropriate authorities and relevant government departments.

 

• As appropriate and relevant, Mix It Up – Live life your way workers will be trained in the responsibility for the obligatory reporting and specified incidents/ serious incidents.

 

• A Participant Incident Database will be maintained.

 

• All participant incidents will be investigated and any remedial actions identified as necessary will be undertaken.

 

• The frequency of specific incidents will be monitored, trends identified and improvement initiatives undertaken as appropriate. Monitoring will be undertaken by the Mix It Up – Live life your way Director.

 

• Participants, carers and workers will be educated about risk minimisation.
   

• Any and all insurance considerations and requirements will be attended to in a timely manner.

 

• Mix It Up – Live life your way acknowledges a duty to deliver services in a safe manner using best practice principles.

 

• Mix It Up – Live life your way recognises the importance of documenting participant incidents in the building of an evidence base that supports reflection and development of better practices and behaviours for safer environments and lifestyles for participants.

• Mix It Up – Live life your way recognises the importance of education in reducing frequency of incidents and minimising the impact of incidents.

 

• All interactions with participants and their advocates in regards to participant incidents will follow related Mix It Up – Live life your way Policy and Procedure.

Review/Consultation

​

The review of this policy is the responsibility of Mix It Up – Live life your way Director This policy will be reviewed at the same time as the corresponding procedure.
