To be encouraged to make your own decisions and choices and be as independent as possible.

To be informed of your rights and responsibilities.

To be supported to connect with other services if needed.

To have access to full community participation.

To have your personal information protected and only used for the right reasons.

To have a person of your choice as your nominated representative.

To have a clearly developed plan of support.

To receive the appropriate support that is of high quality.

To be provided with information about the services policies and procedures

To have your human rights protected and upheld.

To be informed on how to provide us with feedback on our service and how to make a complaint.

To esnure your safety and undertake practices that prevent injury to you and others.

To treat the workers with respect and dignity.

To comply with reasonable requests.

To be responsible for your own behaviour.

To advise the Lifestyle Consultant of any concerns as soon as possible – Providing a quality service to you is our priority.

To advise the Lifestyle Consultant of any health changes or needs prior to the activity or event.

To consider having ambulance cover – in case of an emergency.

To pay for the cost of entrance into an activity or event for yourself and Lifestyle Companion.

To pay your account within 14 days of receiving your invoice.
