This charter represents a commitment from Mix It Up – Live life your way to uphold the rights and needs of individuals using our service.

Participants Rights.

​

  • To be treated with respect and dignity and have your privacy maintained.

  • To be encouraged to make your own decisions and choices and be as independent as possible.

  • To be treated fairly and without discrimination.

  • To be informed of your rights and responsibilities.

  • To be supported to connect with other services if needed.

  • To have access to full community participation. 

  • To have your personal information protected and only used for the right reasons.

  • To have a person of your choice as your nominated representative.

  • To have a clearly developed plan of support.

  • To receive the appropriate support that is of high quality.

  • To be provided with information about the services policies and procedures

  • To have your human rights protected and upheld.

  • To be informed on how to provide us with feedback on our service and how to make a complaint.

  • To esnure your safety and undertake practices that prevent injury to you and others.

  • To provide your feedback or complaints without retribution.

Participants Responsibilities.

​

  • To treat the workers with respect and dignity.

  • To comply with reasonable requests.

  • To be responsible for your own behaviour. 

  • To advise the Lifestyle Consultant of any concerns as soon as possible – Providing a quality service to you is our priority.

  • To advise the Lifestyle Consultant of any health changes or needs prior to the activity or event.

  • To consider having ambulance cover – in case of an emergency.

  • To pay for the cost of entrance into an activity or event for yourself and Lifestyle Companion.

  • To pay your account within 14 days of receiving your invoice.
