Mix It Up – Live life your way is a social connection and engagement service that is focussed on assisting people with complex needs, receiving NDIS funding, to achieve their individual goals by reducing limitations, increase participation and supporting individual hopes, dreams and choices.

​

The office is located in Surrey Downs however, your core work will be undertaken in the broader community providing a variety of services such as: capacity building and assisting clients to participate in community and mentoring opportunities. 

​

To be successful in this role which is a casual position, inclusive of weekends, you must have demonstrated experience in the mental health or disability sector working with clients with complex needs and the ability to develop strong working relationships with colleagues and work independently within a team of productive, collaborative professionals.  

 
Your role will include:
​
  • Providing individual social support to participants, often with complex needs and to undertake a variety of activities of their choices

  • Building rapport with participants and enjoy enabling them to be the best they can be by providing them with mentoring services to assist them to learn new life skills.

  • Developing and following individualised support plans.

  • Documenting accurate case notes and reports as requested.

  • Linking and liaising with a variety of community services.

Essential Qualifications & Experience:
​
  • Certificate III, preference for Certificate IV in the Community / Human Services / Mental Health and a Current Senior First Aid Certificate.

  • Min 2 years’ experience of working with adults with complex needs.

  • DHS Clearances:

  1. NDIS Workers Check​

  2. Working with Children Check

OR​

  1. Disability Services Employment screening

  2. Aged Care Sector employment screening plus national police check

  3. Working with children check

  • Full Comprehensive Vehicle Insurance

  • Current driver’s license and roadworthy vehicle (must pass a safety inspection).

  • Influenza vaccination (or willingness to obtain).

​
