Mix It Up – Live life your way

​

Mix It Up – Live life your way provides a variety of services to people with standard level support needs, assisting you to reach your NDIS and personal goals these include:

  • Support Coordination (Level 2)

  • Support Connection (Level 1)

  • Psychosocial Recovery Coaching

  • Social and Community participation support (accompanying you to participate in social, community and recreational activities of your choice). 

  • Mentoring support (to develop and learn new skills).

Support Connection and Support Coordination 

​

What is Support Connection?

Support Connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.

 

What is Support Coordination? 

Support Coordination – Coordination of supports: This support will assist you to build the skills you need to understand, implement and use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Psychosocial Recovery Coaching

​

Psychosocial Recovery Coaching supports people with a mental health diagnosis to develop skills to manage the complexities and challenges of day to day life. Through building strength, resilience, and personalised strategies to respond to situations in a positive a constructive way that provides a sense of personal control.

Participants are assisted to articulate what a meaningful life means for them and their families and carers, and to then make decisions for themselves. Aiming to increase independence, social participation, and economic participation.

Our Psychosocial Recovery coaches hold a minimum qualification of Certificate IV in Mental Health (or above) or a minimum of 2 years experience in the mental health sector.

Social and Community Participation

​

Participants can choose to be accompanied to a one-off event, short term or long-term activities, and engage a Lifestyle Consultant to accompany you on short-term holiday. We provide 1:1 Lifestyle Companions to participate in out of home events and activities. This is a private and confidential service which also operates after hours and on weekends.

 

Minimum engage of a Lifestyle companion is 2 hours.

Lifestyle Companions accompany participants to a variety of social, community or recreational activities that they would like to participate in outside of their home. For example;

  • Concerts.

  • Sporting events – football, soccer, netball, basketball.

  • Fishing.

  • Movies.

  • Fairs and Markets.

  • Celebrations – Weddings, funerals.

  • Cafes and restaurants.

  • Community clubs and centres.

  • Shopping.

  • Appointments – doctors, court etc.

How does it work?

​

  • You contact the Lifestyle Consultant/Support Coordinator/Psychosocial Recovery Coach and arrange a time to meet (this is a confidential service).

  • You meet with the Lifestyle Consultant/Support Coordinator/Psychosocial Recovery Coach and discuss your needs and desires.

  • You will discuss the fees and charges which relate to your financial situation.

  • The Lifestyle Consultant/Support Coordinator/Psychosocial Recovery Coach will develop an individualised plan with you and will go through the service agreement.

  • The Lifestyle Consultant/Support Coordinator/Psychosocial Recovery Coach will arrange a suitable qualified and experienced Lifestyle Companion to accompany you on your chosen event or activity (for social participation and mentoring activities).

  • The Lifestyle Consultant/Support Coordinator/Psychosocial Recovery Coach can meet with you at your home or at your chosen destination.

  • If meeting at your home – you will travel together either by taxi (participant pays for taxi) or in the Lifestyle Companion’s vehicle (participant may be charged mileage) to your chosen event or activity.(for social participation and mentoring activities).

  • The Lifestyle Companion/Psychosocial Recovery Coach stays with you for the time you have agreed prior to the service being delivered.

  • The Lifestyle Companion/Psychosocial Recovery Coach can assist you to navigate the venue, assist to purchase goods, book the taxi, provide transport, provide stand by personal care support, and provide social support.

  • You are invoiced for the service and payment required within 14 days from the date of the invoice.

Who provides support to you?

​

  • Highly qualified staff who have experience in mental health, intellectual and physical disability, ageing and social inclusion.

  • All employees are engaged by the service, based on their experience, skills, and qualifications.

  • All employees have DCSI clearances – aged, disability/NDIS worker screening check, vulnerable people, and child related.

  • All employee’s vehicles undergo an annual safety check with a licensed mechanic and have full comprehensive insurance.

  • All employees have a current First Aid certificate.

Who can access Mix It Up – Live Life Your Way?

​

  • Anyone aged over 18 who needs a companion and who has standard level support needs.

  • People who want to get out and live life.

  • People who find it challenging to get out on their own.

  • NDIS Participants who have been approved community participation as a core or capacity building support, or

  • NDIS Participants who have been approved for Support Coordination or Psychosocial Recovery Coaching as a capacity building support.

How much does it cost?

​

Our fees are in line with the NDIA pricing arrangements which are reviewed annually. See our Fees & Charges page for detailed pricing. The fees vary depending on the time of day the service is required, as we also operate in the evenings and on weekends.

Things to consider

​

  • Manage medication – we can remind people to take their medication but they must administer their medication independently.

  • Personal care must be managed by the participant – stand by assist only.

  • Physical transfer participants – We have a no lift policy.

  • We do NOT provide services to people under the influence of non-prescription drugs or alcohol.

​
