Mix It Up – Live life your way is a social connection and engagement service that is focused on assisting people to achieve their individual goals. Mix It Up – Live life your way is based in Surrey Downs however, work is undertaken in the broader community, providing a variety of services to people receiving NDIS funding.

The Role

We are currently looking for an experienced Support Coordinator / Psychosocial Recovery Coach to assist and support NDIS participants to build on their own strengths and increase their capacity to be in control of their own lives. Flexible hours will be offered for the role (Casual / Part Time to be negotiated).

​

Key focuses for the role:

  • Working with clients to develop strategies and positive habits for implementation throughout their daily lives. 

  • Discussing progress and strategies with the families and representatives of clients. 

  • Promote the belief that people are the expert in their own lives

  • Help clients find out about different services and supports and how they can help them to achieve their goals.

  • Work in partnership with clinical Mental health services.

  • Maintaining progress/case notes and reporting these back to internal stakeholders.

  • Identifying potential crisis situations and proactively, effectively managing these.

  • Prepare plan reviews and other requested reports to the NDIA and your team leader

  • Help clients better understand the NDIS

  • Acknowledge that each client’s journey will be different

  • Co-ordinate a range of supports including informal, mainstream and funded providers and support participants to complete service agreements where required.

  • Regularly monitor plan outcomes and expenditure to ensure the services remain within the plan budget

  • Strengthen and enhance the client's ability to coordinate supports and self-direct their services in the longer term

 

Skills and experience:

  • Support Coordination experience preferred or transferrable skills

  • Strong, respectful communicator (Excellent verbal & written communication skills)

  • The ability to develop and maintain strong relationships across the sector

  • Excellent interpersonal & listening skills

  • Self-driven, dynamic and responsive

  • Knowledge of the NDIS framework

  • Understanding of the NDIS pricing arrangements and billable hours accountability

  • Strong administrative skills; time management, coordination of tasks, efficient work practices

  • High level of attention to detail

  • Excellent computer literacy

  • Flexible 

  • Ability to work with minimal supervision but seek clarification when needed

  • Established problem solving skills

  • A team player with a ‘Can-do’ attitude

  • Honest & transparent

  • Solid understanding of the Mental Health Sector and Community Resources

  • Provision of satisfactory screening/clearances (NDIS workers clearance or Disability services/aged care & WWCC) or willingness to obtain

  • ‘C’ class drivers licence

  • Full comprehensive vehicle insurance

  • Current influenza vaccination

 

Qualifications

  • Bachelor of Human Services (preferred), however, Minimum of Certificate 4 in Mental Health or Peer Work– essential or 2 years’ experience working with in the Mental Health sector
